Abstract

Spontaneous genital arousal and unwanted orgasms in women are still poorly investigated and may be understood as a multi- factorial syndrome. Many female patients suffer from high levels of distress, even suicidality, and often encounter inadequately informed physicians, which further increases distress, as our case report demonstrates. Some causative mechanisms seem to be understood, such as clitoral priapism, in which an increase in pressure in the erec- tile tissues of the bulbo-clitoral organ causes excitation of the genital corpuscles and Pacinian corpuscles, which can lead to unwanted arousal and orgasms. However, there appear to be factors not only in the clitoris, but also in the pelvic floor area, in nerve conduction, as well as central processing. Patients with persistent genital arousal disorder (PGAD), for example, show spontaneous, intense, and more extensive activation of the para- central lobule even in the absence of genital stimulation. Patients with PGAD also have a high comorbidity of mental disorders such as depression and anxiety as well as borderline personality disorders. Stress has been described as a significant cofactor. Holistic psychosomatic treatment is therefore indicated. Historical examples show that the sociocultural acceptance of female sexuality was largely problematic over centuries. This is all the more true for women with spontaneous genital arousal and unwanted orgasms, which led to even higher social rejection.
